Numbered lists
Unless otherwise specified by client/employer guidelines, use the automated numbering feature of your word processing application to enter numbered lists. (See the specific help guidelines for the program for instructions on how to create a numbered list.)
Some facilities have EMRs or transcription management applications that do not import automatically created numbered lists. Follow account guidelines.
The most commonly used format is to type the number, a period and one space, then begin the text for that item. Do not attempt to align the periods in the numbered list if the list numbers extend to double digits.
DISCHARGE DIAGNOSES:
1. Pneumothorax.
2. Breast cancer.
(etc.)
10. Status post excision and drainage of abscess.
