The question mark is used at the end of a direct question.
Will you have a ride home after the procedure?
Do you have a DNR?
Include a question mark for a question within a direct quote. Put the question mark within the quotation mark only when the question applies to the quoted material.
His first question was, "How long have you had these symptoms?"
The patient asked, "Why am I taking this medication?"
At the end of a sentence, a question mark goes outside the closing quotation mark when it applies to the entire sentence.
How often did the patient say "I'm having chest pain"?
It is common in medical dictation to indicate uncertainty by making the statement "question mark." This should be transcribed using the actual question mark where indicated, not spelling out the words "question mark."
D: Diagnosis question mark possible pulmonary embolism.
T: DIAGNOSIS: ? possible pulmonary embolism.
By the same token, do not use the question mark when the dictator states "question of" or "questionable." These are not questions.
There is a questionable history of abuse.
